Zend Studio will be a good, but it is not free. You can try PSPAD.

vi, with syntax highlighting. Then use a combo of debug messages in my code (that output values that i am working with) and the error log in apache.
personally, i find most GUI IDE's have way to many features. Also i like vi because you can access your shell right from inside the program.
