Uh....

1) Install a Wiki
2) Install a Forum
3) Replace the Wiki's index.php with the Forum's index.php
4) Modify the forum template to point to pages on the wiki.

Sounds like nothing revolutionary. I'm sure it's been done in plenty of places, especially since placing different open-source web solutions on the same site is not uncommon.
