If you want to get this done quickly, I recommend Visual Basic. Pick up a copy of Microsoft Visual Basic 6, and The Complete Idiot's Guide to Visual Basic 6, and you'll be done in no time.

You get the Working Model edition free with the book, which is alright but doesn't let you compile. Of course, if you really want the proper app, there are ways...

A PHP web based interface would be a "free" method to use... although, VB is very nice for quick and dirty GUI apps.

There is always the option of Borland C++ Builder X, you can download the personal edition for free. It is fully functional, and has a complete IDE.. everything you would need to create the said application.
