Here at U of Washington, you gotta have MATH 124-126 (Calc. 1-3) to apply to the CS dept. I took those in high school though :p

After that, math reqs are very little:
MATH 308 (Matrix Algebra)
STAT 390 or 391 (Stats for science or computer science (more specific))

Lots of people around here take an extra 3 classes I think it is for a math minor, and there's also several who double-major (since a few of the 400 level math classes can be counted as CS senior electives).

Oh, and we're quarter-based here too if it matters.
